What cagrilintide is

Cagrilintide is a synthetic, lipidated, long-acting analogue of human amylin, engineered for once-weekly administration. Its backbone borrows from both human amylin and salmon calcitonin, which is the standard route to an amylin analogue that resists aggregation, and it carries a C20 fatty diacid for albumin binding.

Amylin signals through receptor complexes formed between the calcitonin receptor and receptor-activity-modifying proteins (RAMP1, RAMP2, RAMP3). Cagrilintide engages those complexes with a broader profile than a strictly selective agonist would, which is the pharmacological difference between it and newer selective compounds such as eloralintide.[4]

Sequence and structure

A 32-residue lipidated peptide near 4327 Da with a disulfide bridge near the N-terminus. The disulfide is a genuine quality attribute rather than a detail: incorrectly folded or reduced material has the same amino-acid composition and a mass differing by 2 Da, which is within the noise of a careless mass read and entirely resolvable by a competent one.

What the published trials reported

Three published randomised trials define the public record:

  • ·Phase 2, monotherapy. Lau et al., Lancet 2021: a 26-week randomised, double-blind, placebo-controlled dose-finding trial in adults with overweight or obesity, reporting dose-dependent weight reduction against placebo and against liraglutide 3.0 mg.[1]
  • ·Phase 1b, combination. Enebo et al., Lancet 2021: concomitant administration of cagrilintide with semaglutide 2.4 mg, the trial that established the combination now known as CagriSema.[2]
  • ·Phase 2, type 2 diabetes. Frias et al., Lancet 2023: co-administered once-weekly cagrilintide 2.4 mg with semaglutide 2.4 mg in type 2 diabetes, against semaglutide alone.[3]

The limit of the evidence

Cagrilintide has no marketing authorization in any jurisdiction, alone or in combination. Its published record is phase 1b and phase 2, generated with GMP material at defined doses under clinical supervision. Health Canada has authorized nothing containing it.

A great deal of commentary treats the combination results as if they were cagrilintide results. They are not: in the combination trials the amylin analogue is one of two active components, and separating its contribution requires the monotherapy arm.

Verifying a cagrilintide lot

Identity by mass spectrometry against the expected mass near 4327 Da, HPLC purity, and measured content against the labelled amount. For an aggregation-prone class, one additional practical check: material that has partially aggregated may not fully redissolve on reconstitution, and a vial that leaves visible particulate after the recommended reconstitution volume and time is telling you something a certificate issued at manufacture cannot.

What is amylin, and why does it matter?

Amylin is a 37-residue hormone co-secreted with insulin from pancreatic beta cells, signaling satiety and slowing gastric emptying. It is pharmacologically interesting because it acts through a pathway distinct from the incretins, which is why amylin and GLP-1 agents are studied in combination.

Why can native amylin not be used directly?

Human amylin aggregates into amyloid fibrils, which makes it unsuitable as a formulated agent. Analogues in this class borrow structural features from salmon calcitonin to resist aggregation.

How does cagrilintide differ from eloralintide?
